How do I send medical records, imaging, or an insurance card securely?
Submit the referral form and our patient coordinator replies within one business day with a secure upload link. You can also fax records to our office. To protect patient health information, records are never uploaded directly on this page.
Is there any up-front cost to the patient?
For attorney-lien personal-injury cases we can begin care now and bill the settlement later, with no up-front cost. We also accept insurance, HSA/ESA funds, and CareCredit.
